Bristol-Myers Squibb Foundation made diabetes grants available in 2012 to improve health outcomes of people living with type 2 diabetes in the United States. Their “Together on Diabetes” project is now being targeted to four organizations serving seniors, with the help of the United Hospital Fund.
Tested will be a diabetes community control strategy focused on seniors; the first community involved be Washington Heights/Inwood, where the goal is to connect health care and community resources to provide over 1600 seniors living in NORCs (Naturally Occurring Retirement Communities) with the care and supports they need to better control their diabetes.
