The Community-Based Care Transitions Program now includes more than 126 acute care hospitals partnering with community-based organizations (CBO) across 30 sites to provide care transitions services for an estimated 223,172 Medicare beneficiaries residing in 19 states.
These participants will join 7 other community-based organizations already working with local hospitals and other healthcare and social service providers.
On March 14, the Centers for Medicare and Medicaid Services (CMS) announced an additional 23 sites selected to participate in the Community-Based Care Transitions Program (CCTP).
In the first round of awards, the Council of Jewish Elderly SeniorLifewas chosen as one of the Community Based Care Transition Programs. Council for Jewish Elderly in Chicago, IL, is partnering with Northwestern Memorial, Saint Joseph Hospital, and Saint Francis Hospital and working closely with Area Agencies on Aging in Chicago and suburbs, local Care Coordination Units (CCUs), and Illinois’ Quality Improvement Organization, IFMC.
